Please note:

As a beneficiary, your application will be reviewed by the Autism FFCP staff.

  • You will be asked to provide a written statement that explains why you are applying for this benefit.
  • We may ask you to provide more information about your child’s condition.
  • We will need to verify that you are the parent of the child who is receiving the benefits.

  • You are responsible for the cost of any dental care provided to your child.
  • You may also choose to pay the cost of the services out of pocket.
